Home Care Services
Our CareGivers include companion-aides, state certified nurse assistants and home health aides. In addition, Matched CareGivers also has a number of aides trained in specialized care areas.
Services are provided from a 4-hour shift to round-the-clock care.
Care includes but is not limited to:

- Bathing and Dressing
- Exercise and Mobility
- Medication Reminders
- Meal Planning and Preparation
- Companionship and Safety
- Transportation and Errands
- Light Housekeeping
- Linen Change and Laundry
- Special Diet Compliance
- Dementia and Hospice Support
RN Assessment/Case Management
A nurse case manager makes an assessment visit to identify needs and safety issues. A plan of care is developed and reviewed on an ongoing basis to ensure that the appropriate level of care and services is provided.

Clients are visited by the nurse case manager on a regular and as-needed basis to ensure that all those involved in the client's care meet the changing needs of the client.
Other services can include coordination of medical appointment and social activities.
Geriatric Case Management
Registered nurses are available for more complex case geriatric case management on a fee basis.
